Hock Physiotherapy Services
  1. Programme Validity

Unless otherwise stated, all programmes or packages purchased are valid for twelve (12) months from the date of invoice. Upon expiry of the validity period, any unused sessions or treatments will automatically be forfeited. Unutilized sessions are non-refundable, non-transferable beyond the stated terms, and non- extendable.

  1. Transferability of Packages

Purchased packages are transferable to a maximum of four (4) immediate family members only. Each transferred customer must undergo an initial consultation, subject to a RM50.00 consultation fee, payable on the first visit.

  1. Package Upgrade

Customers may choose to upgrade their purchased package before the final session is utilized and within the validity period. Upon upgrading, the new package validity will be six (6) months from the date of payment for the upgraded package. Package upgrades are optional and without obligation.

  1. Refund Policy

All payments made for programmes, packages, treatments, complimentary services, and products are strictly non-refundable.

  1. Appointment Arrival Time

Customers are encouraged to arrive at least ten (10) minutes prior to their scheduled appointment time. Hock Physiotherapy Services reserves the right to reschedule appointments for late arrivals, and treatment time will not be extended in such cases.

  1. 6. Appointment Cancellation / Rescheduling

Cancellation or rescheduling of appointments must be made at least twenty-four(24) hours in advance. Failure to provide the required notice, a no-show without prior notification, or cancellation for invalid reasons will result in the scheduled session being considered as attended and deducted from the customer’s existing programme.

  1. Professional Discretion

The attending physiotherapist reserves the right to suspend, modify, or discontinue any treatment programme if it is deemed clinically inappropriate, unsafe, or not in the best interest of the customer’s health or recovery.


 
Medical Liability Disclaimer

Physiotherapy treatment involves manual therapy, exercise therapy, electrotherapy, and other rehabilitation techniques, which may carry inherent risks including, but not limited to, temporary soreness, discomfort, fatigue, or aggravation of existing symptoms.

While all treatments are performed by qualified physiotherapists with professional care and in accordance with accepted clinical practice, Hock Physiotherapy Services shall not be held liable for any injury, adverse reaction, or complication that may occur as a result of treatment, except in cases of proven negligence.

Customers are responsible for informing the physiotherapist of their complete medical history, including any existing medical conditions, injuries, allergies, medications, or contraindications that may affect treatment.


 
Treatment Consent

By attending treatment sessions at Hock Physiotherapy Services, the customer acknowledges and agrees that:

  • They have been informed about the nature, purpose, and potential benefits of physiotherapy treatment.
  • They understand that no guarantee or assurance has been made regarding treatment outcomes.
  • They voluntarily consent to receive physiotherapy assessment and treatment provided by the attending physiotherapist.
  • They understand that they have the right to ask questions, request clarification, or withdraw consent to treatment at any time.
