Frailty can be identified when at least three of the following are present:
- Decline in lower limb function: slower walking / reduced grip strength / unable to perform sit-to-stand > 5 times
- Weight loss (commonly >4 kg within one year)
- Reduced energy levels: easily fatigued / lack of motivation
